Patients are responsible for understanding their medical insurance plan policies!
Patients are responsible for obtaining insurance referrals from their primary care physician if required by their insurance plans, prior to their appointment. If a patient arrives for an appointment and we have not received a referral, he/she may need to reschedule the appointment.
To schedule an appointment, have your insurance card and the name of your primary care physician and call: 212-689-9587 for our office at 71 Park Avenue or 212-683-6073 for our office at 36A East 36th Street.

CONFIRM YOUR APPOINTMENT:
We will call or email, if we have your address, 1-2 days prior to your appointment to confirm your appointment. If we cannot reach you, we will leave a message. Please call back to confirm your scheduled appointment.

If you come without these items, you may need to reschedule your appointment.

- Your referral, or confirm that it has been received by our office at least one day prior to your visit.

Co-payments are expected at the time of visit.
If possible, please fax all relevant prior medical records to be put in your chart.
CANCELLATIONS AND RESCHEDULING
When canceling or rescheduling a regular appointment, please contact the office at least 1 business day prior to the scheduled appointment date.
When canceling or rescheduling a surgical appointment, please contact the office at least 3 business days prior to the scheduled visit. Failure to do so will result in a $250 cancellation fee.
When canceling or rescheduling a Mohs surgical appointment, please contact the office at least one week prior to scheduled appointment. Failure to do so will result in a $500 cancellation fee.
