Insurance & Referrals
Patients are responsible for understanding their insurance coverage and plan requirements. If your plan requires a referral, you must obtain it from your primary care physician before your appointment.
If you arrive without a required referral (or we have not received it), you may need to reschedule.

What to Bring
If you arrive without required items, you may need to reschedule.
All patients should bring:
- Insurance card
- Referral (if required) — or confirm our office has received it at least 1 day prior
New patients:
- Completed Patient Intake Form
Returning patients not seen in over six months:
- Completed Patient Information Update Form
Co-Pays
Co-payments are due at the time of your visit.
Medical Records
If possible, please fax any relevant prior medical records so we can add them to your chart.
Cancellations & Rescheduling
Please notify the office as early as possible if you need to cancel or reschedule.
- Regular appointments: at least 1 business day notice
- Surgical appointments: at least 3 business days notice
- Late cancellations may result in a $250 fee
- Mohs surgery appointments: at least 1 week notice
